Azra AI Acquires Blackford Analysis to Close the Most Dangerous Gap in Healthcare: What Happens After the Scan

One platform now carries every imaging finding from the first scan to completed care, ending the era of AI flags that go nowhere

NASHVILLE, Tenn., Oct. 20, 2026 /PRNewswire/ — Every day, scans reveal signs of cancer, heart disease, and other serious illnesses that never turn into care. Azra AI, the industry-leading end-to-end enterprise platform that identifies patients who need care in real time and manages their journey from early suspicion through survivorship, today announced it has completed its acquisition of Blackford Analysis, a pioneer in enterprise AI orchestration for medical imaging.

Blackford Analysis puts imaging AI to work on the medical image, and Azra AI carries every confirmed finding through to completed care. For the first time, health systems have one partner from the first scan through survivorship. Azra AI works alongside the electronic health record, pairing imaging findings with EHR data such as genomic testing, labs, patient history, appointments, and clinical documents. Care teams get a complete view of each patient, from screening and incidental findings to progression modeling, tumor board, registry, and clinical trials. With one contract and a single IT connection, health systems can now find patients earlier and keep them on the right path, whether navigation is handled by their own teams or by Azra AI.

Azra AI acquired Blackford Analysis from Sopris Capital, Azra AI’s parent company and majority owner, which acquired it from Bayer on October 2, 2026. The acquisition comes less than six months after Azra AI acquired Thynk Health, the specialized leader in lung cancer screening and incidental findings management.

Where patients fall through the cracks

When a radiologist mentions a suspicious nodule, mass, or a cardiac finding, and the report lands in the electronic health record. No one clearly owns the next step, and many of the tools meant to track these patients are fragmented, so navigators end up managing follow-up by hand in spreadsheets and notes and focused on a single point in the care process.  Published research shows that fewer than half of patients with an incidental finding on CT receive the needed follow-up to the completion of their designed care journey, and those who do often wait weeks or months for care that should take days.

These point solutions have made the problem worse. Many imaging AI companies focus on a single condition, such as lung, and present it as the complete package. Those tools stop at the finding, leaving the patient in limbo. EHRs are built to order, administer, and document care, not to follow patients from the pixel to the result, through treatment planning and clinical trial matching, and through treatment itself with care navigation and multidisciplinary meetings, all while capturing the data required for registries and accreditation. The result is a patchwork of narrow tools, limitation by current capabilities and regulatory clearances which create care gaps where patients fall through the cracks.

From the first image to completed care

The Blackford Platform offers more than 130 AI applications from over 45 partner vendors through a single, vendor-neutral platform. Once Blackford applies the correct AI to the correct case and a report is generated, Azra AI steps in and combines each report with the patient’s history to identify who needs care in real time and routes them to the right team. The platform supports all screening and incidental findings, complex care navigation for oncology and other clinical disciplines, multidisciplinary meetings, cancer registry automation, and clinical trial matching and research. Every patient is tracked through to completed treatment. No spreadsheets. No lost patients. One install. One platform. Providing enterprise scale and standardization across every health system in the enterprise.

Growth at enterprise scale

Azra AI is deployed at 1,500+ sites, including five of the top 10 U.S. health systems. The company has grown 11x in the 28 months since John Marshall became CEO in mid-2024, who brought in one of the strongest executive teams assembled in AI and health tech. Marshall will serve as CEO of the combined companies. Blackford Analysis will continue to serve its existing OEM partners and health system customers, with no disruption to current deployments, and all Azra customers will now have the Blackford platform available to them.

“Every health system wants to catch disease earlier. The harder part, and the one our industry hasn’t connected, is making sure what gets caught turns into immediate care,” said John Marshall, CEO of Azra AI.  “Today a patient can have a finding documented in a report and still fall through the cracks, and in cancer, that lost time can cost a life. Many imaging AI companies have chosen to focus on a single condition, such as pulmonary nodules, but patients don’t arrive with just one, and health systems need a platform that follows every finding to care. Adding Blackford to Azra gives health systems a single platform that connects imaging AI from across the industry with agentic care coordination. This is the change health systems and patients have been waiting for.”

“Our mission has always been to make imaging AI simple to adopt and valuable at scale,” said James Holroyd, Managing Director of Blackford Analysis. “Joining Azra AI carries that mission beyond the reading room. Our partners and customers keep the vendor-neutral platform they rely on today, and they gain a direct path from AI-detected findings to coordinated patient care.”
